Registration

1. Download Registration Form.
2. Download Release Forms.
3. Download Room Assignment Form.
      Note: There are three release forms*:
      1. General Form
      2. X-Treme Fun Release Form
      3. Rock Climbing Release Form
      *We must have at least the General Form for the teen to be able to attend the conference…the other forms are optional depending upon the teen’s desire to participate in those events.
4. Collect $30 registration fee per teen.
5. Collect Registration and Release Forms from teens.
6. Fill out Room Assignment Form. (Note: We must know who will be in each room …this is a Westin Hotel Policy and it is a Fire Department Code)
7. Mail forms and one check payable Tri-City Baptist Church to:
      Rocky Mountain Youth Conference
      C/O Tri-City Baptist Church
      6953 West 92nd Lane
      Westminster, CO 80021
8. Park van or bus on east side of the Westin and come to registration table in east lobby at the Westin Hotel between 4:00 – 6:30p on April 5th with balance check ready when you check in. Do not go to the hotel Check-In desk!
